[receive] terminate_stream() identifies discovered sources by name but sequence tracking by CID

Universe discovery tracking is keyed by source name, but sequence tracking is keyed by CID.
If two sources share the same name, this can result in:

terminated universes on the wrong discovered source record, and/or

merged discovery pages incorrectly across different CIDs.

Fix: track discovery sources by CID
