Rare file attachment issue permission issue

If a file attachment is linked to two answers and one is `inactive` then students will not be able to see the file attachment of the active one.

This occurred in one class for one student `INC2277805` `INC2276833`

currently not sure how an answer used the same file attachment twice but setting the inactive answer's file_id to null solved the issue for the students
